Billing & Insurance
Christina Cid Speech Therapy is in-network with Medicare B and Blue Cross Blue Shield.
I also accept private-pay clients and can provide a Superbill for potential out-of-network reimbursement through your insurance provider. I strongly encourage you to review your plan's out-of-network benefits to understand your coverage and your responsibility for therapy services.

Referring Providers:

Patients/Caregivers:
If your doctor has ordered a speech-language pathology evaluation, please call (405) 256-3544 or email christina@cidspeechtherapy.com to schedule your appointment.
If you are not being referred by a physician, call today to discuss whether you could be a good treatment candidate. I can help you contact your physician to obtain a referral if it is deemed appropriate. An evaluation appointment will be scheduled when the necessary documentation is received.
Prior to your appointment, I will need:
A copy of your insurance card
Your past medical history or a recent physician's note
Your physician referral form (usually faxed directly to me by your physician)
